Our Community Mental Health Goal

The goal of psychosocial rehabilitation is to teach emotional, cognitive, and social skills that help those diagnosed with mental illness live and work in their communities as independently as possible.

Targeted Case Management (TCM)
Excellent Medical Center LLC will assume all TCM cases in the Miami Dade County area on Adult appellation and continue helping our community. Services provide case management to adults with a serious mental illness to assist them in gaining access to needed medical, social, educational, and other services.
Case Management is to help older adults and adults with disabilities access and use health and community-based long-term care services. These services help people maintain independence at home and maximize their well-being.
Our Targeted Case Managers are community advocates who will interview you to identify your strengths and needs, then come up with a plan to address those needs. They will help determine the appropriate services available in your community to address those needs.
